Enrollment Policies

2008 Enrollment Packets are distributed during the interview process.

Our Parent Handbook is the best source for all policies and procedures
The first step to enrollment in Child's Play Day Nursery is the call to set up an interview.  Interviews are conducted in the evening and
are by appointment only.  

After our initial meeting, and if you are interested in placing your child in our program.  
You will be given an enrollment packet.  It will
contain our most recent Parent Handbook, a contract and the following forms, that will need to be filled out and returned by your child's
start date.

Health History/Addendum          Emergency Contact Sheet
Permission Form                         All About My Child Form
Immunization Form                     Indoor/Outdoor Activity Form
Picture/Video Permission
Water Sprinkler Play Permission (seasonal)
Sunscreen/Insect Repellent Form (seasonal)  

Our Parent Handbook is complete with all policies and rules of CPDN.                  
   
You will be expected to leave a security deposit (equal to 1 week's tuition fees) for immediate
enrollment and a
4-week holding fee for delayed enrollments.  This deposit will serve as your last  
two weeks of paid childcare.  After the first 20-business days (trial period), a 3-week's notice must
be given.  If not, you forfeit your security deposit.
